Thomas began hormone replacement therapy in May 2019 and came out as a trans woman to her coaches, friends, and the University of Pennsylvania’s women’s and men’s swim teams during her junior year.

While undergoing hormone therapy, Lia was required to swim for the men’s team as a junior in the 2019–2020 academic year, and then swam for the women’s team in 2021 after taking a year off school to maintain her eligibility to compete while competitive swimming was canceled due to the COVID-19 pandemic. Thomas has complied with all gender-related policies in order to be allowed to compete in NCAA swimming as a woman.

Thomas’s distance swimming event progression peaked in 2019, with a drop in times during the 2021–22 season. Her sprint swimming event progression showed a dip at the start of the 2021–22 season before rebounding in 2021 with near-lifetime bests in the 100 free and a lifetime personal best in the 50 free.

In 2021, primarily conservative media outlets, including Fox News, began extensively covering Thomas. Anonymous parents of University of Pennsylvania swim team members wrote to the NCAA in early December, requesting that Thomas be declared unable to participate.

Thomas began swimming short distance sprints in the 2021–22 season (after not swimming any 50 free or 100 free events from 2016 to 2021 or any 100 free races from 2019 to 2022). She set the top university women’s team times in the 50 free, 100 free, 200 free, 500 free, 1000 free, and 1650 free during the 2021-2022 season.

Thomas finished sixth in the 100m freestyle race at a match against UPenn’s Ivy league foe Yale in January 2022, losing to four cisgender women and Iszac Henig, a transgender male (transitioning without hormone therapy). 

Thomas was dubbed “the most contentious athlete in America” by CNN in February 2022, and “the face of the debate about transgender women in sports” by Sports Illustrated in March 2022.

As a sophomore on the men’s swim team in 2018–2019, Thomas finished second in the Ivy League finals in the men’s 500, 1,000, and 1,650-yard freestyle.

After winning the women’s 500-yard freestyle with a time of 4:33.24 in March 2022, Thomas became the first openly transgender athlete to win an NCAA Division I national championship in any sport; Olympic silver winner Emma Weyant finished second, 1.75 seconds behind Thomas.
